Attendee Registration Fees
If you need to update your contact information, add items to your current registration, invite a colleague to the Annual Meeting, or print another payment receipt, please use the Registration Resource Center or call 1-800-748-5064.
Includes badge, printed program, all sessions and regular courses, entrance to the Exhibit Hall, ASOA Celebration and the Film Festival Awards Ceremony.
There is an additional fee for the ASOA Optional Workshops, ATPO COA/COT/COMT Review Day, COE Review Day, COE Exam, Cornea Day, Glaucoma Day and ASCRS Skills Transfer Sessions.
Information on ASOA Optional Workshops and ASCRS Skills Transfer Sessions will be available online and in the Preview Program.
ASOA Optional Workshops are separate registrations. You do not have to be registered for the meeting to attend these courses
Registrants of ASCRS and ASOA programs are welcome to attend either or both Symposium and Congress programs. The 2013 meeting will also include a joint registration category for those interested in attending both the ASOA and Technicians & Nurses programs.

Crossover between the ASOA and Technicians & Nurses program will be permitted to those registered in the joint registration category only. Those registered for either the ASOA or Technicians & Nurses program will not be allowed to go between the two programs. Registrants of either category are permitted to attend ASCRS course/paper sessions/symposia.
Registration Categories
- ASCRS Member: An MD, PhD, or DO who is a current member of ASCRS. Registration fee does not include annual membership dues. (NOTE: An MD, PhD, or DO who is a member of ASOA only MUST join ASCRS and pay appropriate membership dues.)
- Resident/Fellow: An MD or DO who is CURRENTLY in a residency or fellowship program. A current letter of verification from department chairperson or fellowship sponsor must accompany this registration along with a picture ID and proof of date of birth. The age limit to be considered a resident/fellow is 35 years. Letters and credentials, which will be verified, can be faxed along with your registration to 703-434-3004. Your registration will not be complete until your residency/fellowship documentation has been received. All residency/fellowship documentation must be received by April 12, 2013 to be eligible for the discounted rate.
- Young Physician: A domestic MD or DO who is in the first two years of practice. To be eligible, an ophthalmologist must have been an ASCRS Resident/Fellow member and become a Young Physician Member immediately upon completion of training.
- Corporate/Other: Manufacturers and other corporate personnel who do not have exhibit space but wish to attend the Symposium and Congress. NO solicitation is permitted. This includes attendees who plan on attending the exhibit hall only. Physicians cannot register in this category.
- ASOA Member: Office administrator or other who is a current member of ASOA. Physicians cannot register in this category.
- ASOA Nonmember: Office nonclinical staff who are not current members of ASOA. Physicians cannot register in this category.
- Technicians & Nurses: Ophthalmic assistants, technicians, technologists and nurses program. Physicians cannot register in this category.
- Integrated Eyecare Optometrists: Optometrists (ODs) can register under this category only and must certify one of the following:
- Employed by an ophthalmologist
- Employed by a medical school (not a college of optometry)
- Employed by a managed-care provider
- Employed by the military
- Employed by industry/manufacturer
- Employed by a corporate center (TLC, LCA, etc.)
Attendance at or participation in ASOA Congress or ASCRS Symposium programs, events, courses, presentations, or other activities does not qualify an optometrist to perform surgery, does not qualify as continuing medical education, and does not justify claims to surgical expertise or licensure.
- Spouses/Guests: Guests 21 years of age and over are required to pay a registration fee. Fee includes badge, printed program, entrance into the exhibit hall, ASOA Party and the Film Festival Awards Ceremony. This badge is limited to spouse, guest, relative, or other individual accompanying an ASCRS or ASOA member. Members, potential members, or staff will not be considered under the spouse/guest registration category. Physicians/ASCRS•ASOA members cannot register in this category. (Entrance into sessions/courses is not included.)
Membership Dues
Registration fee does not include annual membership dues. NOTE: If you are registering as an ASCRS or ASOA member, the appropriate membership dues will be added to your registration fee. New or renewing ASCRS or ASOA members, please add appropriate membership dues.

Membership dues must be current through April 30, 2013 to receive member registration rates. If you do not know your membership expiration date, please call our office at (703) 591-2220.
Badge Information
Badges and meeting materials will not be mailed. Please bring your confirmation email with you on site to retrieve your badge.
All lost badges are subject to $160 badge reprint fee.
Payment
Registrations received without payment will not be processed.
Payment (in U.S. funds) can be made by personal check, bank draft, wire transfer, travelers check, VISA, AMEX, MasterCard or Discover.
Note: Bank processing fees for wire transfers or bank drafts must be included in the payment for completion of registration. All funds must be received by April 12, 2013 or you must register and pay onsite.
Cancellation/Refund Policy
All cancellations and requests for refunds must be in writing and received no later than March 1, 2013. A handling fee of $125.00 will be deducted from each cancelled registration. The membership portion of the registration New/Renew fee is nonrefundable. NO REFUNDS will be given after March 1, 2013.
Program Participants
All program participants are required to register and pay the appropriate fee for the meeting.
