Speaker Ready Room

Mandatory Check-In for all ASCRS presenters in Speaker Ready Room (Room 200-212, Moscone Convention Center) All presenters of ASCRS courses, papers, and symposia must check in at the Speaker Ready Room 24 hours before their session. Check-in is required even if you have pre-submitted your presentation. Speaker ready room staff will have information on any room or time changes in the program. Presentations must be submitted on USB Memory Stick. All presentations must be created in PowerPoint. If you use Keynote, you must be prepared to export your file(s) to a “Clickable” QuickTime video (this is an option in Keynote) that will be embedded into a PowerPoint. The speaker ready room does not have internet access for downloading your presentation or videos from the web.
ASOA and T&N course instructors do not have to check in at the Speaker Ready Room (Walnut Room, Marriott Marquis) but are encouraged to use the Speaker Ready Room to review their presentation(s).
Disclosure of Unapproved/Off-Label Use
If your presentation concerns the use of a drug or device that has not been approved by the U.S. Food and Drug Administration or concerns off-label use of a drug or device approved by the FDA for other use, you must disclose that fact during your presentation.
Submission Withdrawals
If an author/instructor/producer is unable to present, he or she must notify ASCRS/ASOA by e-mail to submissions@ascrs.org or by facsimile to (703) 547-8846. If an author/instructor/producer fails to notify ASCRS/ASOA of withdrawal or does not present, he or she will be ineligible to submit in 2014 (see Guidelines for Nonacceptance).
Guidelines for Nonacceptance
- Submissions that are commercially biased will not be accepted.
- If the author/instructor/producer/photographer did not present his/her scheduled presentation the previous year and did not follow the withdrawal guidelines, the current year’s submission will not be accepted.
- If the submission information was previously published or presented elsewhere, the submission will not be accepted.
- If the program participant has not successfully completed the CME Faculty Review and test, the submission will not be accepted.

Copyright Assignments

By submitting this presentation (paper, poster, course, film), I agree to grant and assign exclusively to the American Society of Cataract and Refractive Surgery and The American Society of Ophthalmic Administrators (“the Societies”) all of rights, including copyright, in the presentation listed below, as well as any related materials used for audiovisual presentation (collectively referred to as “the works”). I also grant and assign to the Societies the rights to film, record, reproduce, reprint, distribute, sell, and otherwise make use of the works, or any presentation of the works, in any media or format, and authorize the Societies to use my name, likeness, photograph, and biographical data in connection with its use and promotion of the works. I understand that I will receive no royalty or other monetary compensation from the Societies for this assignment of rights and the subsequent use of my works by the Societies.
I represent and warrant that the works are my own original work or that I have obtained all necessary permissions and authorizations from all individuals or entities that may otherwise have a right, title, or interest in the works; that I have full right and power to make this assignment; that the works do not violate any copyright, proprietary or personal rights of others; and that the works are factually accurate and contain no matter libelous or otherwise unlawful.

By execution of this agreement, I hereby accept the ASCRS/ASOA invitation to participate as a speaker at the ASOA Congress on Ophthalmic Practice Management conference to be held on April 19–23, 2013 (“Conference”), and agree to abide by the terms and conditions set forth herein. If I am unable to fulfill my speaking commitment or am unable to participate for any reason, I will notify ASCRS/ASOA in writing not less than 30 days prior to the Conference. I further agree to provide ASCRS/ASOA with a copy of all materials I intend to use as hand-outs as part of the presentation prior to the deadline date.
By my signed acceptance of the terms and conditions of this agreement, and in consideration of the opportunity to speak and present my work at the Conference, I hereby grant the American Society of Ophthalmic Executives, a division of the American Society of Cataract and Refractive Surgery, a royalty-free, perpetual worldwide license to copy, reproduce, film, distribute and/or sell the work in any media or format in connection with the Conference or sale or other distribution of Conference materials, as well as authority to use my name, likeness, photograph, and biographical data in connection with its use and promotion of the works. I understand that I will receive no royalty or other monetary compensation from the Societies for this license of rights and the use of my works by the Societies in connection with the Conference. ASCRS/ASOA will not use the works for any purposes unrelated to the Conference or sale/distribution of Conference materials without my written permission prior to use.
The Societies understand and agree that the above grant of rights does not constitute a transfer of copyright and that I remain free to present the materials or revised versions thereof elsewhere for any purposes.
